APPOINTMENTS
Our office is open for outpatient appointments 8:00 AM – 5:00 PM
Monday through Friday, Saturdays 8:00-1:00.Please schedule
appointments and place non-emergency calls, including inquiries about
medications, during regular business hours. Our staff is efficient,
friendly, courteous and ready to help you with health insurance
claims, appointments, and much more.
INSURANCE
Payment for office services is due when the service is rendered. We
accept cash, personal check, money order, or credit card (Visa, Master
Card). Insurers require that we collect co-payments. Full payment of
applicable co-pays, deductibles and co-insurances is due at the time
of the service. Our office will file insurance claims on your behalf.
If you prefer to file your own claims, we will provide you with an
itemized bill.
PRIVACY NOTICE
Physicians of the North Shore adheres to the privacy, security and
electronic transactions guidelines that are established in the PRIVACY
POLICY of Physicians of the North Shore and that are addressed by the
Health Insurance Portability and Privacy Act of 1996. The Health
Insurance Portability and Accountability Act of 1996 (HIPAA) mandated
regulations that govern privacy, security, and electronic transactions
standards for health care information. These regulations require major
changes in how health care organizations handle all facets of
information management, including reimbursement, coding, security, and
patient records. Physicians of the North Shore complies with
established regulations that govern patient confidentiality and
privacy and remains abreast of pending new regulations as they are
proposed and modified. You will be asked to sign authorizations,
consents, and releases in accordance with our Privacy Policy. We
require a copy of the Power of Attorney document for any patient who
has assigned a POA.
Our Privacy Policy is posted in the office. We urge you to read it
carefully. You may request a copy of the Privacy Policy at
registration.
